What is an ACO?

  • Accountable Care Organizations (ACOs) deliver care through a model that promotes shared goals among value-based healthcare collaborators.
  • Often times, ACOs are physician-led and participate in the Medicare Shared Savings Program (MSSP) offered through the Centers for Medicare and Medicaid Services (CMS).
  • MSSP establishes incentivized, performance-based payment models for qualifying healthcare organizations serving traditional Medicare beneficiaries that promote collaborative and efficient care delivery.
  • This care delivery model mitigates fragmented and insufficient access to healthcare while reducing unnecessary costs and improving patient outcomes.

Our ACO

UF Health ACO Gainesville, LLC was formed in 2024 to further promote the advancement of medicine and the continuation of the high-quality care provided to our patients. The goals of the UF Health physicians pertaining to the ACO are:

  • Improving clinical outcomes for all patients
  • Reduce healthcare costs for patients, employers and health plans
  • Enhance coordination of care
  • Further the advancement of evidence-based care protocols

Our Vision

To serve as a national model for collaboration among physicians, healthcare providers, and the communities they serve to achieve the best possible population health at the best value.

Questions?

Please use our contact form in the For patients and participants section.

Organizational Leadership

The ACO is directed by a board of physician representatives from the UF Health practice and a member from the Gainesville Community.

Independent doctors are not employed by UF Health, but may provide medical care at one of our locations.

For Patients and Participants

We provide coordinated care for you to get well and stay well.

We aim to provide patient-centered care focused on individual patient needs. Healthcare providers coordinating your care can see the same test results, treatments, and prescriptions—more coordination helps save time, money, and frustration by avoiding repeated tests and appointments.

When you choose a healthcare provider participating in an ACO, they will help you get the right care at the right time. You can visit Medicare.gov and log into (or create) your secure Medicare account to choose a primary care doctor.
