Advance Care Planning Facilitator Certification Courses
Honoring Choices Florida® is a community wide advance care planning (ACP) program coordinated by Community Hospice & Palliative Care. The ACP Facilitator Certification Course provides the learner with the skills and tools to have an advance care planning conversation and help individuals Discuss, Decide, and Document their health care decisions. We will define advance care planning and identify ways to assist those you serve in reflecting on their values, beliefs, experiences, and wishes, while documenting decisions.
If you work with individuals who could benefit from discussing, deciding, and documenting decisions about the care they would want when faced with a sudden or serious illness, and you would like to learn skills on how to guide that conversation, register now for this Advance Care Planning Facilitator Certification Course.
Nurses, LCSW’s, LMHC’s, and LMFT’s earn 6.5 CE’s for on-line pre-class work and 8.0 CE’s for classroom instruction. CE Provider No. 50-420
Cost
Registration is $150.00 and includes on-line pre-class work, all day virtual instruction, material, and continuing education hours.
