“ Dr. Staud seems to be knowledgeable and open to my input regarding treatment; however, I do not feel he has listened to my limitations as he refused to assist with renewing my work accommodation paperwork even though I have had the accommodation with my employer for 3 years and it was has allowed me to maintain employment. ”
