Douglas Williams Executive Health Program is now located within UF Health Internal Medicine Clinic at Springhill Building 2. We are located on NW 86th Terrace, on the North side of NW 39th Avenue, approximately ¾ of a mile east of I-75 at Exit 390.
Parking
Once you’ve turned into the Springhill NW 86th Terrace drive, go straight through the roundabout and continue to follow signs for Springhill 2. Keep going forward once you see the sign for Springhill 2 and you will also pass the signs for valet. Keep going straight and you will come to a dead end with a sign labeled FUTURE THRU STREET. PLEASE PARK IN FRONT OF THESE SIGNS.

Participants in the Douglas Williams Executive Health Program are asked to complete a medical history questionnaire to bring to their appointment.
The new patient questionnaire is for new patients, but also for returning patients who have not been to Executive Health in the past 5 years.
