Description
UF Health Orthopaedics and Sports Medicine Institute will be providing pre-participation/sports physicals to area youth participating in athletics for the 2017-2018 school year.
- All forms must be signed by a parent/guardian prior to physicals.
- Forms can be found on the FHSAA website at FHSAA.org. (Click on the EL2 Form under For Students and Parents)
