Before your baby is born
Pre-register with UF Health Flagler Hospital’s Labor and Delivery
We recommend that you preregister with the hospital by your fifth month of pregnancy. This will ensure that we have all of your vital demo graphical information needed in order to verify your insurance coverage and health benefits.
- Please complete the following form. After we receive this information, we will create a MyChart account for you.
- You will receive an email on how to sign in.
- We will send you your pre-registration paperwork in MyChart.

On the day of your visit
Please remember to bring your insurance card and a photo ID with you on the day of your visit. All patient responsibilities are due at the time of service. Your insurance coverage and health benefits will be verified in advance of your delivery.
