UF Health Primary Care – West Augustine will open in May 2026.
A primary care clinic in West Augustine focused on expanding access to high-quality, compassionate health care. This location is part of a broader effort to create a community-centered health and wellness destination in partnership with local organizations to address long-standing gaps in access and health outcomes.
In addition to primary care services, this location also provides access to UF Health St. Johns Care Connect and the UF Health Behavioral Health Resource Center. These services help patients navigate care, connect with behavioral health support and access additional health resources when needed.
Choosing a doctor is an important decision. At UF Health Primary Care, our providers are here to treat you when you’re sick—but more importantly, we are here to help you and your family stay well. Our preventive care, behavioral health, disease management and wellness services are designed with your long-term health in mind.

Costs, billing, and insurance
