Getting an Autism Spectrum Disorder (ASD) assessment

Get a Referral From your Primary Care Provider

You'll need a referral for a Comprehensive Autism Diagnostic Evaluation (CADE) directed to the Center for Autism and Neurodevelopment. Your primary care physician or pediatrician can provide you with a referral.

Referral Letter and Forms

When we receive the referral, we will send you a referral letter. The letter will explain how to fill out and return the necessary forms. Once you submit the forms as instructed in the referral letter:

  • The patient navigator will call you to answer any questions you may have.
  • Our department schedulers will call to schedule an appointment. Appointments are in high demand, so please plan accordingly.

Forms

We will not process the forms unless we receive the referral.

Be sure to mark all information you approve to be shared. You'll see a highlighted yellow box about our case conference. In a case conference, your health care team will discuss the strategy to best provide you care. This team includes: 

  • psychologists
  • rehabilitation therapists
  • child psychiatrists
  • patient navigators
  • other autism specialists

Case conferences allow us to deliver faster care to our patients with the greatest needs.

Post-Evaluation

You will receive recommendations from your provider during your visit. After your appointment, your patient navigator will call you. They will discuss available resources to act on those recommendations.
