Getting an Autism Spectrum Disorder (ASD) assessment
Get a Referral From your Primary Care Provider
You'll need a referral for a Comprehensive Autism Diagnostic Evaluation (CADE) directed to the Center for Autism and Neurodevelopment. Your primary care physician or pediatrician can provide you with a referral.
Referral Letter and Forms
When we receive the referral, we will send you a referral letter. The letter will explain how to fill out and return the necessary forms. Once you submit the forms as instructed in the referral letter:
- The patient navigator will call you to answer any questions you may have.
- Our department schedulers will call to schedule an appointment. Appointments are in high demand, so please plan accordingly.
Forms
We will not process the forms unless we receive the referral.
- Developmental history: for children from birth - 17 years old
- Developmental history: for adults 18 years old and above
- Release of information form
Be sure to mark all information you approve to be shared. You'll see a highlighted yellow box about our case conference. In a case conference, your health care team will discuss the strategy to best provide you care. This team includes:
- psychologists
- rehabilitation therapists
- child psychiatrists
- patient navigators
- other autism specialists
Case conferences allow us to deliver faster care to our patients with the greatest needs.
Post-Evaluation
You will receive recommendations from your provider during your visit. After your appointment, your patient navigator will call you. They will discuss available resources to act on those recommendations.
