Preparing for Your Visit

Prior to arriving at the hospital, you may be required to follow special instructions given to you by your physician or healthcare provider. It is important that you follow these instructions and any pre-operation guidelines.

Here is a list of important things you should bring, when you arrive at your UF Health St. Johns scheduled appointment:

• Medication List
• Insurance Card
• Driver’s License/ Picture ID
• Patient financial responsibility
• Order for test(s)

Your insurance card and photo ID are vitally important. All co-payments, deductibles and patient responsibilities are due at the time of service. Your insurance coverage and health benefits will be verified in advance of your appointment.
