At UF Health Shands Rehab Hospital, a variety of professionals who specialize in rehabilitation help patients learn the skills they need to adapt to life’s changes. Patients work closely with physiatrists (physicians specially trained in physical medicine and rehabilitation), therapists, rehabilitation nurses and psychologists so they can regain as much independence as possible, as quickly as possible.
The purpose and goal of rehabilitation is to make it possible for patients to return to their homes with strong support systems made up of family, friends, and others in their communities.
Once a patient is admitted to UF Health Shands Rehab Hospital, therapy needs are determined by rehab specialists. Working closely with you and your family, the team, which includes physiatrists, rehab nurses and therapists, will establish your initial goals and a treatment plan to achieve them. During the course of your care, these goals may be revised to reflect your progress and personal choices.
The length of your stay (LOS) in the rehabilitation hospital is based on progress made and goal achievement. When further progress toward the established rehabilitation goals is unlikely, can be achieved in a less intensive setting or you have met your goals, you will no longer meet criteria to remain in the rehabilitation program. You will receive ongoing information regarding the LOS throughout your stay.
A registered nurse will complete an evaluation of your condition and together with the rehabilitation physician and other professionals, a decision is made about your admission to the rehabilitation hospital.
The rehabilitation team will begin your evaluation the first day of admission. Once the evaluation is completed, the nursing and therapy staff will then help you identify rehabilitation goals based upon their evaluations and your expectations. Your rehab team will meet weekly to review progress toward your established goals and update those goals and length of stay as necessary. Your case manager will communicate any changes to the established plan regarding your length of stay and your projected functional status goal at discharge.
During your stay at UF Health Shands Rehab Hospital, the team providing your care will be led by a physician specially trained or experienced in rehabilitation medicine. You will be seen by your physician on a daily basis.
The treatment team develops individualized treatment plans based on your specific needs. Treatment activities will be geared toward achieving your goals.
