UF Health St. Johns Care Connect is an outreach program helping area residents connect with the resources they need to live a happy, healthy life.
We can help. There are many programs in the community to keep you and your family safe and healthy – it’s just a matter of connecting with us at Care Connect to get started. We know the process can be difficult to navigate, and our role is to close this gap so we can support you through all of life’s issues, including the challenges and the good times.
Get connected to help: Follow these three easy steps
Connect with a Community Health Associate
A dedicated Community Health Associate will call and email you to complete an intake within five business days. This helps us understand your unique needs.
Receive your personalized resources
Receive personalized referrals to resources and services designed to support you and your family.
Reimagining wellness in your community
Care Connect is dedicated to connecting individuals to the resources they need, especially when they may not know where to turn for help. Below are some ways we can assist:
Healthcare
We connect you to healthcare services such as primary or specialty care, affordable insurance options and medical vouchers for those who qualify.
Dental
Our team can connect you to routine and emergency dental services.
Prescriptions
We help you navigate programs available to offset or cover prescription costs.
Food assistance
We increase food accessibility through local food pantries, food delivery programs and enrollment in SNAP benefits.
Housing
We navigate the resources available for emergency, transitional and long term housing.
Mental health
If you or your child needs support with mental health services, we find providers to fit your needs.
Rental and utility bill assistance
Residents are referred to local programs to help with rental assistance and past due utility bills.
Transportation
Do you need healthcare, but have no car? We provide rides to and from important medical appointments.
What we do
At UF Health St. Johns Care Connect, we’re proud to offer physical, mental, and social support wherever you are. The goal is to improve the health and wellbeing of our overall community by addressing social and environmental factors that can get in the way – these are called the social determinants of health.
Without addressing these unmet social determinants of health, the impact on our health can be significant. For example, poor health is closely connected with inadequate housing, food insecurity, and unemployment. This leads to a snowball effect of increased spending on healthcare expenses and long term health issues. But it doesn’t have to be that way.
