View and Share Medical Records Using myUFHealth

  • Log into MyUFHealth
  • Click on Menu in the upper right
  • Scroll down to Sharing in the menu
  • You can make a Record Request or Share Records

Once you send in the form, your records will be released within 96 hours.

Request Medical Records

You can download the form or call our office at (352) 594-0909 to have the form mailed or faxed. If you need copies of actual videos or tracings, please contact the department that performed the test.

Once you have completed the form, you can:

  • Fax it to (352) 627-4500, or
  • Mail it to PO Box 100348, Gainesville, FL 32610

Medical records are mailed, emailed or released to MyChart per your request.

Non-UF Health Shands Doctor’s Offices

If the request is for a non-UF Health Shands doctor’s office/facility, please ask the doctor’s office to do the following:

  • Use their office fax cover sheet;
  • Write your name, date of birth and what they need on the office fax cover sheet;
  • Write STAT at the top of the page;
  • Fax the form to the ROI office at (352) 265-1098.
  • The ROI office will fax the records directly to your doctor’s office.

Record Access Fees

You may review your medical record by appointment only. The review fee is $25.00 per hour. Under Florida law you may be charged a fee of up to $1.00 for every page copied (plus tax and handling). You can save money by asking for specific documents or a record abstract (all recent major reports) rather than the entire record. The copying fee is waived for copies provided directly to a healthcare provider for continuing medical care. Most record requests are fulfilled within 5 to 7 business days.
