UF Health’s Pediatric Care Coordination Program (PCCP) is a family-centered, community-based model for the care of children with complex medical and social needs.

Patients enrolled in the PCCP have multisystem organ disease, are dependent on medical technology or have a history of frequent hospitalizations. The PCCP was designed to give children with complex medical and social needs more accessible outpatient health management resulting in less ER visits and hospitalizations and more time at home, to heal, grow, and develop.

PCCP offers the following:

UF Health PCCP team: A subset of providers within the General Pediatrics Department consisting of a pediatrician, two Advanced Care Practitioners, RN Health Coach, and clinic staff experienced with medically complex patient needs.

Provider availability: Patients, families, home health nursing, specialists, insurance case managers, and medical equipment providers have direct access via team phone and fax lines to the PCCP providers during office hours. This is to expedite communication of care needs whether this is a quick medical question, care plan clarification, home health orders, etc.

Clinic access: Patients and families are given extended primary care office visits with acute walk-in clinic availability to see PCCP providers every weekday, excluding rare exceptions for holidays. This is in addition to the existing UFHealth resources including pediatric after-hours clinic and after hours on call services.

Specialist coordination: PCCP has access to all UFHealth specialists to aid in referrals, appointments, and communication with specialists.

UF Health Pediatric Hospital: Although PCCP aims to reduce preventable ER visits and hospitalizations, sometimes these are necessary for appropriate care whether for emergency issues or planned procedures etc. The PCCP outpatient clinic is positioned just a few blocks from the hospital, and the team is available for consultation with the hospital providers.

Education: PCCP aims to augment patients’ and families’ understanding of their complex medical needs, devices, medications, and care plans. As a part of the UFHealth organization PCCP also takes part in training the next generation of health care professionals to provide the best patient care.

Make a referral

This is the criteria for the UF Pediatric Care Coordination Program. Physician referral is appropriate if the patient demonstrates one or more of the following:

  • Two or more body systems with significant chronic conditions (significant chronic condition is defined as a physical, mental or developmental condition that can be expected to last at least a year, will use health care resources above the level for a healthy child, require treatment of control of the condition, and the condition can be expected to be episodically or continuously debilitating)
  • A progressive condition that is associated with deteriorating health with a decreased life expectancy in adulthood
  • Continuous dependence on medical technology, or expected, for at least 6 months
  • Frequent or prolonged hospitalizations and/or Pediatric Emergency room visits

And meets all criteria below:

  • Caregiver willingness for the Pediatric Care Coordination Program to assume primary care of the child, to demonstrate appropriate behavior towards medical team and supportive staff and try to attend appointments in a timely manner or notify team of rescheduling needs.
  • Demonstrates a need for care coordination. For example, difficulty scheduling appointments, referrals, resources, orders, home health services, care access.
  • Primarily uses UF Health Pediatric Specialists for care
  • Has reliable transportation and a working telephone service
  • Agrees to discuss and pursue transition back to General Pediatrics or Adult Primary Care once deemed appropriate by PCCP Medical Director or sooner if desired by patient and family.
