High school graduate? Congratulations on your achievements. As you prepare for your next journey, the Pediatric Pulmonary Asthma Program would like to wish you the best of luck in your future endeavors.

We also would like for you to take a brief moment to be sure that you feel comfortable with handling asthma on your own. Do you have all the tools you need to care for yourself when you are away from home? Do you have any questions for your doctor? Are you ready to transition to an adult health care provider? If you have any questions, you can: discuss with your heaIthcare provider at your clinic visit, send a question through MyUFHealth or contact primary care office at (352) 265-22.22. If you are a pulmonary patient, you may call at (352) 273-8380.

Remember when you tum 18, it is up to you to make decisions for your own healthcare, but you can ask others for help. If you need help, your doctor's office may ask for a form to be completed that allows who you would like to see your records.
