Choose the Option That Works for You
Option 1: Consultation Only — $199
Already have pharmacogenetic test results? Great — you can get started right away.
- Upload your existing test results and a list of your current and past medications through our secure patient portal.
- Meet with one of our pharmacists over video to go over your results and build a personalized medication plan to share with your doctor.
Option 2: Testing and Consultation — $599
Don't have test results yet? No problem. We'll take care of everything.
- We'll send you a DNA test kit performed by UF Health Medical Laboratories.
- Once your results are ready, you'll meet with one of our pharmacists over video to review your results and create a personalized medication plan for your doctor.

Using Your Own Test Results? Here's What to Know
We can only accept test results from approved, certified laboratories. If you already have results, contact us and we'll let you know if they qualify.
The lab must be CLIA-certified, and the specific genes and variants tested must meet our quality standards.

Insurance coverage
Insurance coverage for DNA testing varies. Here's what you should know:
- Contact your insurance company directly to ask about coverage. You may need to give them the test code: CPT 81418.
- We accept certain insurance plans. If yours isn't accepted, you'll pay the cash price and receive an itemized receipt you can submit to your insurance company yourself for possible reimbursement.
- Need a medical claim form filled out? Just email us a blank form from your insurance company at MyRx@cop.ufl.edu and we'll return it to you within one week.
- If you don't have insurance, you'll be responsible for the full cost.

Appointment policy
If you need to cancel or reschedule, please give us at least 36 hours' notice. If you miss three or more appointments without notice within a three-month period, you may be removed from our program.
Refund policy
This policy is effective as of August 23, 2022 and applies to all orders.
All sales are final and are not eligible for refunds.
- Damaged test kit? Take a photo of the damage and email it to MyRx@cop.ufl.edu within three days of receiving it. If approved, we'll send you a free return label and replace your kit.
- Kit lost in the mail? Email us at MyRx@cop.ufl.edu and we'll send you a replacement at no charge.
- Missed appointment? Refunds are not provided if you are removed from the program due to missed appointments.
