Transitioning from Cancer Treatment
Through the UF Health Adult Cancer Survivorship Program, patients can receive the care they need and learn the skills required to maintain optimum health and continue to lead fulfilling lives. Our collaborative partnership with patients and caregivers provides support and follow-up for a higher quality of life after cancer treatment ends. You do not need to have been treated at UF Health in order to be seen in our Adult Cancer Survivorship Program.

What is Cancer Survivorship?
Successful cancer treatments (radiation, chemotherapy and surgery) and the effects of disease often impact cancer patients long after their treatment is over, and may require lifelong follow-up care. Through clinical care, education, counseling, surveillance, and coordination of care, Cancer Survivorship programs utilize a personalized approach to improve the health and quality of life for patients.
Making the Transition
Cancer patients generally progress through three phases, and our programs are available to those who have completed cancer treatment and have begun what is known as “transition”:
- Diagnosis and cancer treatment
- Transition: after cancer treatment ends through approximately two years (when the Survivorship Program will begin to care for and support patients)
- Long-term survivorship care
Getting Started
The adult survivorship program was created to focus on the medical and psychosocial issues of adult cancer survivors through exceptional multidisciplinary care. The team provides the following upon a patient’s first visit to the survivorship clinic:
- A review of past treatment, a screening and health evaluation, and a discussion of current problems or concerns.
- Enrollment in the UF Health Adult Cancer Survivorship Care Plan, based on information related to diagnosis, anticipated long-term problems, and recommendations for patients and their primary care providers.
The Benefits of Adult Cancer Survivorship Care
After the patient’s first visit, the survivorship team will recommend a follow-up schedule. The team will collaborate on the patient’s progress and monitor for late effects.
The UF Health Adult Cancer Survivorship Program team will also initiate referral to sub-specialists as required, including:
- Physicians
- Advanced practice providers
- Nurses
- Social workers
- Nutritionists
- Physical therapists
- Speech language pathologists
- Dentists
- Swallowing specialists
